I've recommended sinus surgery to over three hundred patients in my career. And for years, I believed it was the right thing to do. Open up the blocked passages, and the symptoms go away. That's what every ENT is taught. But after enough years in practice, something started to bother me. Patients I operated on would come back months later—still unable to breathe clearly, still waking up congested, still living on steroid sprays. The CT scans looked better. The suffering didn't. And no one could explain why. I remember one patient clearly. A middle-aged mother of two named Jennifer. She'd spent seven years battling chronic sinusitis, trying every medication, every rinse, every specialist in town. She came to me completely blocked. Constant facial pressure. Classic surgical candidate. We did the procedure. It went smoothly. Eight weeks later she came back and said, "I still can't breathe through my nose. I thought this would fix it." I told her to give it time. That post-surgical swelling can take months to fully resolve. Four months later, she said she still felt the same crushing pressure behind her eyes every morning. That was the day I started questioning everything I thought I knew. If blockage was the real cause, surgery should fix everyone. But it didn't. So why did so many people keep suffering even after we made "more space"? That question kept me up at night. I started reading every new study I could find. Late nights at my desk.
